Lasting Power of Attorney

This is a legal document administered by the Office of Public Guardian. If you are above the age of 21 years, you can voluntarily appoint someone whom you trust to make decision on your behalf should you lose the capacity to make your own decisions. It allows surrogate decision making in aspects of personal care (which includes making decisions for medical issues as well) and welfare as well as property and financial aspects.

The LPA certificate must be issued by a lawyer, psychiatrist or other certified medical practitioners.
