The Advance Medical Directive is a legal document which one can sign in order to choose not to have life sustaining measures initiated should one be terminally ill.
It can be signed, in the presence of 2 witnesses, by anyone aged 21 years and above and who has the mental capacity to do so.
It is mandatory that one of the witnesses MUST be a registered doctor. The other witness must be over 21 years in age.
